Our Northwest Family Clinics Patient Portal makes it easy to:
- Communicate with your doctor
- Receive messages from your doctor
- See lab results
- View upcoming appointments
- Schedule appointments
- Review and renew medications
- Manage a family member’s health
To sign up:
- Go to Northwest Family Clinics home page
- Click “Patient Portal” at the top of the page
- Click “Login/Sign-up”
- Scroll down this page and click “I need to Sign up”
- Then complete the requested information form. Sign HIPAA agreement. Create your username and password.
- Type in “Northwest Family Clinics” as your health care organization in the search field. Then choose Northwest Family Clinics (any location) in Search Results box. Click “Connect”. Then click “Next” to move to the next page.
- Finally, click “I Accept” to approve your request for portal access.
We will receive your information and portal request and complete the rest of the process. Your portal account should be up and running within 2 business days, likely sooner.
